Our home care offering to our residents is paramount to our philosophy of ” living in your home as independently and as long as possible.” Living Choice Australia has been an aged care approved provider since 2014 and is seeking to grow its delivery of home care across all of its villages.
As a Home Care Case Manager you will hold overall responsibility for identification, general assessment and management of home care packages & unfunded delivery of care to our residents whilst working closely with our Executive Care Manager & General Manager in the deployment and expansion of home care.
